The incident occurred on June 12. As per the report, the Boeing 787-08 Dreamliner, operating as Air India flight AI 171 from Ahmedabad to London, crashed merely 32 seconds after takeoff.

The AAIB report states that the switches responsible for supplying fuel to both engines were turned off, which caused confusion in the cockpit.
Citing the cockpit voice recorder, the report reveals that Captain Sumit Sabharwal asked his co-pilot Clive Kundar, “Why did you shut off the engine fuel?” To which Kundar responded, “I didn’t.”

The 15-page AAIB report confirms that both engines suddenly shut down on their own just seconds after takeoff. Right after the aircraft reached a maximum indicated airspeed of 180 knots, the fuel cut-off switches for Engine 1 and Engine 2—responsible for sending fuel to the engines—moved from ‘Run’ to ‘Cut-Off’ position.
As the fuel supply stopped, the N1 and N2 speeds of both engines began dropping rapidly. The pilots were unaware that the fuel switches had been turned off.
